SUMMARY DESCRIPTION OF WORK Professionals at this level have excellent communication skills with clients, family members and community stakeholders. Specific project, program or office responsibilities may occur at this level. Decision-making, problem solving, and highly developed interpersonal skills are critical. Heartland Family Service is implementing a Certified Community Behavioral Health Clinic which is designed to provide a comprehensive range of mental health and substance use disorder services to vulnerable individuals.The Coordinator will lead the case coordination effort for the Certified Community Behavioral Health Clinic (CCBHC) including training HFS staff involved in care coordination and supervising CCBHC case coordinators. This person is active in the community, partners with other agencies, and looks for opportunities to help clients fill gaps in their care. Work Schedule: Monday through Friday, 8:30 a.m. to 5:00 p.m. Essential Duties and Responsibilities

  • Develop training protocols for CCBHC care coordination including standards of care across programming.
  • Hires, supervises, monitors, trains, evaluates and develops CCBHC Case Coordinators.
  • Participates in interdisciplinary team meetings to obtain information, provide information, and assist with service planning
  • Leads the care coordination team in providing trauma-informed care to clients. Sets the tone for a safe, enriching environment for clients and staff.
  • Ensuring daily coverage and responsibilities are completed as required. Assist with conducting case coordination duties as part of the team
  • Able to evaluate program services and make recommendations
  • Ensure data quality standards are consistently achieved
  • Understand, monitor and provide process oversight to high risk clients
  • Works independently, with little supervision, and as a part of a team
  • Completes outreach and enrollment-related paperwork to high level of detail
  • Provide presentations and initial contact with businesses, care providers and other professional entities as program outreach as needed
  • Networks with community providers and other agency programs in order to assist clients in accessing needed community resources.
  • Ensure required client and program information is collected, entered in prescribed formats, and reported in a timely manner
  • Supervises the completion of intake/discharge/data collection information with clients.
  • Displays a courteous and caring attitude at all times to the clientele, volunteers, and visitors of the agency.
  • Cooperates and collaborates with program area staff, volunteers, and other Agency staff.
  • *Is dependable and punctual regarding scheduling and attendance.
  • Abides by all specific program and Agency procedures, policies, and requirements.
  • Develops personal and program related skills through participation in internal and external
  • Training opportunities including printed material and audio and/or visual media.
    • Create, maintain and share as appropriate a dynamic self-care plan.
    • Strive to make connections between the agency and the larger community whenever possible in order to contribute to the agency's ongoing fundraising and friend-raising efforts.

        • Bachelor's Degree in a related filed required, Master's Degree preferred
        • Three years' experience in Behavioral Health.
        • 1 year of experience in supervising staff.
        • Valid driver's license, acceptable driving record. COMPETENCIES Leadership- Good managers should be able to lead the employees they manage.
