Conduct patient assessments and evaluate their needs and living environments. Develop and implement individualized care plans that focus on achieving realistic patient goals. Carry out therapeutic interventions to improve patients’ functional abilities. Educate families and caregivers on patient care interventions. Regularly document and report on treatment and progress. Collaborate with other healthcare professionals to optimize patient care.
