Job Description:

home healthcare services, is seeking a compassionate and driven
Home Health Physical Therapist to join our team. This full-time
position offers a $7,500 sign-on bonus and is geared towards a
professional looking to make a significant impact in the lives of
patients by providing physical therapy services in their homes. The
ideal candidate will be responsible for assessing patient needs,
crafting personalized care plans, and implementing therapy
interventions to help patients achieve their maximum levels of
function and independence. Conduct comprehensive patient
assessments to determine functional impairment and potential.
Develop and implement tailored care plans based on each patient’s
needs and doctor’s prescriptions. Document patients’ progress and
adjust care plans as needed. Educate patients and families on
treatments and home exercises. Maintain patient privacy and
confidential patient information. Collaborate with other healthcare
professionals to ensure the highest quality of care. Participate in
continuing education to improve patient care. Strong interpersonal
and communication skills Ability to work independently and manage
time effectively. Critical thinking and problem-solving skills.
Proficient in using healthcare software for patient documentation.
