Position Overview In this role, you’ll use your nursing expertise to provide direct client services that improve the health of pregnant and postpartum individuals across Pottawattamie, Mills, Cass, and Shelby counties in southwest Iowa. The position is based in Council Bluffs, IA —just minutes from downtown Omaha—and offers a flexible, community-focused schedule. Pay range: $30 – $33.50 per hour ( $2/hr bilingual differential for English/Spanish) Schedule: Full-time | Hybrid model with up to 20% remote work Paid time off: Up to 41 days annually (holidays, vacation, and wellness) Key Responsibilities Conduct home visits to deliver prenatal and postnatal education, assessment, and support. Develop and evaluate individualized care plans; provide case management as needed. Educate the community about available maternal and family-health resources. Maintain timely and accurate documentation in all required systems. Collaborate with the Director of Public Health Services to ensure compliance with Iowa HHS and Medicaid standards. Build strong, trust-based relationships with families, caregivers, and community partners. Travel throughout southwest Iowa to provide home-based and community services. Additional Duties Support other Public Health functions and Mobile Wellness Unit events. Communicate effectively and always maintain confidentiality. Occasionally work flexible hours (evenings or weekends) based on client needs. Seek ongoing opportunities for professional growth and development. Qualifications Active RN license (Iowa or Compact State). CPR/First Aid certification. Valid driver’s license and proof of insurance. Strong organizational, documentation, and communication skills. Preferred: Experience in public health, home visitation, OB, or community nursing. Bilingual (English/Spanish) preferred – additional $2/hour.
