DHHS Nebraska is seeking a dedicated health educator to enhance
access to diabetes self-management, education, and support programs
and improve the quality of care among healthcare providers to
improve outcomes for Nebraskans with diabetes. Job Duties: Improve
Access to and Participation in Diabetes Education Programs – The
coordinator will play a pivotal role in enhancing access to
Diabetes Self-Management Education & Support (DSMES) programs. This
involves ensuring that more individuals with diabetes have the
opportunity to engage with and benefit from these programs, which
are critical to improving long-term health outcomes. Enhance the
Quality of Diabetes Care Among Healthcare Providers - Emphasize
improving the quality of diabetes care delivered by healthcare
providers. The coordinator will work closely with healthcare teams
to improve their diabetes management practices, potentially through
training, sharing best practices, and incorporating evidence-based
strategies into care routines. Manage Federal Grants and Ensure
Compliance – Oversee the successful implementation and
administrations of federal grants. This includes developing grant
applications, budgets, and work plans, ensuring that the programs
meet specified goals and objectives, and maintaining compliance
with federal and state regulations. The coordinator will also
evaluate performance measures to assess the program’s
effectiveness. Foster Collaborative Relationships with Partners –
The coordinator is expected to build and maintain relationships
with internal and external partners, such as healthcare providers,
local health departments, and community organizations. This
collaboration will involve identifying community resources and gaps
in diabetes care and working together to address those gaps and
improve program outcomes. Provide Technical Assistance and Training
– Another key objective is providing technical assistance and
training to a variety of stakeholders, including contractors,
healthcare clinics, and local health departments. The goal is to
equip these groups with evidence-based strategies, policies, and
practices that will improve diabetes management and nutrition
programs in the community. Contract and Subaward Oversight – This
role oversees the development, implementation, and monitoring of
contracts and subawards. The coordinator will ensure that these
agreements adhere to federal and state regulations, and that
partners are fulfilling their obligations in a way that supports
the broader goals of the diabetes management and education program.
Requirements / Qualifications Minimum Qualifications : Bachelor’s
degree or equivalent coursework/training in community health
education, biological science, adult/continuing education or
related field AND experience in public/community health education
activities with responsibility for developing, evaluating, and
leading health education program activities; assessing health
needs; writing and designing educational and informational
material, training aids, and presentations; preparing grant
proposals; or planning and leading group sessions. Related
experience may substitute for a bachelor’s degree on a
year-for-year basis. Preferred Qualifications: Master's degree in
public or community health, public administration, healthcare,
nutrition, dietetics, or related field. Minimum of 3 years'
experience working in public or community health, public
administration, healthcare, nutrition, dietetics, or related field.
Supervisor and/or leadership experience and experience managing
federal grants and contracts/subawards. Knowledge, Skills, and
Abilities: Knowledge of: The principles and practices of public
health planning and implementation; the principles of grant
writing, reporting, and grant administration. The 10 essential
public health services; contract and subaward writing, monitoring,
and management. Skills: Supportive supervision, including use of
progressive discipline to support teammate success the practices;
and techniques of developing partnerships and collaborations.
Abilities: Plan, assign, direct and evaluate the work of
subordinate staff; maintain accurate and thorough records; prepare
grant proposals and reports; interpret and apply federal grant
requirements, agency rules, policies and procedures; Communicate
effectively; design educational and informational materials,
training aids, and presentations; plan and lead group sessions;
prioritize and manage time. Problem solve and make decisions;
balance needs, requirements, and workload for multiple projects. If
